Waiver of Liability
I hereby acknowledge that I am voluntarily participating in dog training with Punk Paws, LLC. Instructors will exercise all necessary and proper care and precautions to keep all participants and dogs safe during training to prevent injury to any person, property, or other animals. However, because animals may be unpredictable, Punk Paws, LLC cannot protect against every potential injury, risk, or accident.

I understand that by attending dog training classes I may be putting myself, my family, my dog(s), my other animals, and/or any guests who may also attend, in a situation not without risk. Some of the dogs that I/we may be exposed to could be hard to handle and could cause injury even when handled with the utmost care. I will assume all risk of any injury or damage resulting from the actions of any dog including my own while in class, on the grounds of Punk Paws, LLC property, or any surrounding area when attending any training function. I understand that I am responsible for my dog being current on rabies, distemper, and kennel cough vaccinations. I understand that any time my dog is in contact with other animals it may expose him/her to minimal risk of communicable disease. I understand that there may be saliva, water, food, and/or other debris present in the training area. I hereby waive and release the instructors, Punk Paws, LLC, and Erin D. Ganzel from all liability for injury, damage, or illness that I or my dog may incur while attending the training classes.

I hereby consent to my dog being the subject of photographs and/or audiovisual recordings and authorize Punk Paws, LLC and Erin D. Ganzel to use the mentioned to be exhibited for educational purposes or marketing tools.

I acknowledge that I have had full opportunity to discuss all concerns I have regarding foregoing risks with Punk Paws, LLC and its authorized representatives.

I represent that I am at least 18 years of age.
Informed Consent
1. Sessions/classes need to be paid in full before the first class.

2. Dogs showing signs of serious behavioral issues should not be put into group classes. It is your responsibility to notify the instructor prior to class if you believe your dog may have such to discuss options. The instructor reserves the right to remove your dog from class without a refund if your dog’s behavior becomes disruptive or dangerous to the class.

3. You are required to clean up after your dog. Dogs must be kept on leash unless otherwise instructed.

4. You acknowledge your dog has never showed signs of aggression towards other dogs or humans.
​
5. The instructor reserves the right to cancel class due to inclement weather. Cancellations will be rescheduled at the instructor’s discretion.

6. It is advised small children do not attend class because they may be at an increased risk of injury and/or may be a distraction to the owner/handler. Children under 12 may attend class with adult supervision but may not handle dogs during training class.

7. You acknowledge your dog’s vaccinations for distemper, parvo, and rabies are up to date (with the exception of puppies under 12 weeks of age). Bordetella (kennel cough) is recommended but not required.
